P0171 and P0174 together (both banks lean). Classic Toyota V6 intake manifold gasket failure. Shop confirmed with smoke test. Gasket set was $45, labor was $280. Code hasn't returned in 3 months and 7,000 miles. If you have a Toyota V6 around 100k miles and get both lean codes simultaneously, it's almost certainly the intake manifold gasket.

P0171 on my N20 BMW. Turned out to be the crankcase vent valve (CCV) - plastic housing cracked and was sucking unmetered air. BMW part was $120, took about an hour. The car immediately idled smoother after the fix. Very common on these engines around 60-80k miles.

Dirty MAF sensor on my EcoBoost. Spent $8 on CRC MAF cleaner, cleaned it, reset the code. Lasted 3 weeks before P0171 came back. Bought a new Motorcraft MAF for $85 on Amazon. No more lean code, fuel economy came back up almost 2 MPG. Should have just replaced it first.
