Pennsylvania is one of the strictest inspection states. Every registered passenger vehicle needs an annual safety inspection, and 25 counties (mostly the metro Philadelphia, Pittsburgh, Harrisburg areas) also require an emissions inspection on the same visit.

Safety inspection is typically $35-$50. Emissions inspection (in 25 counties) is another $30-$45, so combined you often pay $65-$95. Repairs are extra.

Every 12 months. Your sticker shows the month it expires. You can inspect any time during that month.
In an emissions county, an active CEL is an automatic emissions fail. Diagnose the code, fix it, drive a complete drive cycle, then return.
PA offers a $150 emissions repair waiver if you have spent at least $150 on a documented emissions repair and still fail. Keep all receipts.
Driving with an expired inspection sticker is a fine up to $25 plus court costs, and a moving violation. After 4 months expired you can also be required to retest from scratch.
