Ohio is one of the easier states. There is no statewide safety inspection at all. The only inspection is the E-Check emissions test - required every two years in 7 northeast Ohio counties (Cuyahoga, Geauga, Lake, Lorain, Medina, Portage, Summit). The test is free at official E-Check stations.
Rules change. Verify the current Ohio requirements with the state DMV or environmental agency before you go. This guide reflects the program as of 2026.
| Item | What the Inspector Checks |
|---|---|
| OBD-II scan (1996+) | Check engine light status, stored codes, readiness monitors. This is the primary test. |
| Gas cap pressure test | Tested for proper seal on most vehicles. |
| Visible smoke check | A quick visual for blue or black smoke from exhaust. |
| Pre-1996 vehicles | Older gas vehicles get a Two-Speed Idle (TSI) tailpipe test, free at E-Check stations. |
| Diesel and hybrids | Diesel-powered vehicles and hybrids over 8,500 GVWR are also tested. |
| No safety check | Ohio does not check brakes, tires, lights, suspension, or windshield in inspection. |

No. Ohio does not require any periodic safety inspection. Police can still cite you for broken lights or bald tires, but there is no annual sticker.

Free at official E-Check stations. You pay nothing for the test. Repairs to pass are at your expense.
Every two years, in alignment with your registration renewal. New cars are exempt for the first 4 model years.
You can retest free as many times as needed. Ohio also has a hardship extension program if repairs would be a financial burden.
Yes. Battery-only EVs are exempt from E-Check. Hybrids with a gas engine are still tested.
